Request Accommodations and Services
Students who wish to disclose a disability and discuss appropriate academic accommodations designed to create equal access to educational opportunities, need to:
- Complete and return an SCTC Verification of Eligibility for Accommodations Form requesting needed accommodations. Forms are available by contacting the Counselor for Students with Disabilities (jjacobsonberg@sctc.edu).
- Provide documentation of the disability to the college. Documentation must be recent, appropriate and relevant information that provides specific diagnostic information, and a diagnosis of a disability that causes a substantial limitation to a major life function. The documentation must clearly substantiate the need for all of the student's specific accommodation requests and be signed by an appropriately licensed professional in the specialty area. These documents are held in a confidential file in order to help us better meet your support service needs.
- Return documentation and Verification/Request form to: Judy Jacobson Berg, Counselor, SCTC, 1540 Northway Drive , St. Cloud , MN 56303.
- Schedule an appointment to meet with the Judy to review your plan for utilizing appropriate accommodations. Call 320-308-5089 or 1-800-222-1009 voice or 320-308-5988 (TTY).
Appropriate and reasonable academic accommodations are detemined on an individual basis. Accommodations must be specific to the disability need. Sufficient advance notice is required by qualified students when requesting accommodations. In accordance with the Americans with Disabilities Act (ADA), accommodations will not be provided 1) for personal daily living devices or services even though the individual may be a qualified individual with a disability, or 2) that result in a fundamental alteration in the nature of a service, program or activity or in undue financial or administrative burdens. Denial of requested accommodations or services can be appealed by contacting the Office of the Vice President of Academic and Student Affairs.
